Instructions for 2016 Form 40 with Calculations


The Form 40 has been enhanced to complete all calculations and to compute the amount of tax due. Just key in your data prior to printing the form.

IMPORTANT: If you choose to use the fill-in option, please do not handwrite any other data on the form other than your signature. Also, do not attach the pre-printed label to the form. It will cause problems with processing. Your information will be included in the 2-D barcode.

  • Requirements for downloading forms

    You MUST have a copy of Adobe Acrobat Reader 11.0 or later installed on your computer in order to view and print these forms. You can download a copy from the Adobe website. It is also necessary to use either Internet Explorer 10.0 or later versions in order to accurately view or download these forms.

  • Calculated Fields - The fields highlighted in yellow are read-only fields and no data can be entered in these fields. Data can be entered in the fields shown in white.
  • Fields Not Required - If you have a field that you do not need to file out, please leave the field blank and do not enter "NONE" or "N/A" in the field.
  • Pre-printed Label - Do not attach your pre-printed label to the form. This information is included in the 2-D barcode information. If you attach the pre-printed label it could cause problems with processing your return.
  • Interest and Dividend Income - If you have Interest and Dividend Income less than $1,500, you still need to fill out the Schedule B. Click on the GO TO SCHEDULE B Button and enter your information. Then press the Return to Page 1 button to return to the First Page.
  • More than Four Employers - If you have more than four employers, you need to attach a schedule and use the same headings for the Wages section (list the employer, Alabama tax withheld and Alabama wages) for each W-2. This schedule can be on paper or a computer spreadsheet. On the Form 40, Page 1, Line 5a, write "See Attached Schedule" and record in Columns A and B the total amount of the Alabama wages and Alabama tax withheld from all the W-2s.
  • More than One Schedule D and/or Schedule E - If you have more than one Schedule D or Schedule E, complete an additional Schedule D and Schedule E for the total number of Schedule D and/or Schedule E needed and print them out. At the bottom of the Schedule D and Schedule E, check the "More than one Schedule D/Schedule E" checkbox. Enter the Total amount of Income/Loss from all the Schedule D's on line 3 and/or the Total amount of Income/Loss from all the Schedule E's on Line 6. Attached ALL the Schedules D and E to the printed return.
  • Navigation Buttons - The buttons in Blue are there to navigate to the different pages. You also can move to the different screen by using the navigation buttons available in Acrobat and Acrobat Reader.
  • Printing Form - You must use the green PRINT FORM button to print your form. If you try to print the form using the Print command in Acrobat or Acrobat Reader, you will get one page with a statement and eight blank pages.
  • Alabama Income Tax Withheld from 1099R or W-2Gs - If you have Alabama income tax withheld from your 1099R or W-2G, enter the tax withheld on line 5, Column A of Form 40, Page 1 and enter a zero in Column B for Income. Enter the income amount of the Pension/Annuities on lines 5a and 5b of page 2, Part I of the Form 40. Enter your Gambling Winnings on line 8 of Part I, Page 2 of the Form 40. IRA distributions are reported on lines 4a and 4b of Page 2, Part I of the Form 40.
  • Schedule CR - If you worked in more than one state, you must use the Schedule CR Worksheet. Check the box stating that you are using the Schedule CR Worksheet. This will take you to the Schedule CR Worksheet. Enter the information for each state that you worked in and are claiming Credit for Other States. The total will automatically calculate and post on the Schedule CR.

    2D Barcode - The Form 40 has also been enhanced to print a two dimensional (2D) barcode. The PRINT FORM button MUST be used to generate the (2D) barcode which contains data entered on the form. The use of a 2D barcode vastly improves processing of your return and reduces the costs associated with processing your return.

  • Direct Deposit of Refunds - The Direct Deposit option is available on this version of the Form 40. Please make sure you check the box on the bottom of Page 1 and enter your Routing and Account Number Information. Please double-check your routing and account number information before you print your form. In order to use the Direct Deposit option, you must print the form with the 2-D barcode. Any returns submitted without the 2-D Barcode will be processed manually and a paper check will be sent.
  • Saving Form - If you have Adobe Reader 11.0 or the complete version of Acrobat, you will have the ability to save your form. If not, you will not be able to save your return. You cannot save the return in Acrobat Reader in version 10.0 and lower. Therefore, please make sure that you print a copy for your records.
