Account Number (Column 1) – Enter the Alabama withholding tax account number. The account number will be either a ten digit number or it will begin with an R followed by 9 digits. Account numbers 700000 and above will begin with an R followed by nine digits, R000700000. Account numbers 699999 and below will be 10 digits with leading zeros, 0000699999 . This is a Required field.

Period End (Column 2) – The format for this field is CCYYMMDD = century century year year month month day day. Enter the period for which the return is being filed. If the return is for a month (Jan, Feb, Apr, May, July, Aug, Oct, Nov), enter the ending date of that month. For example January 2009 should be entered as 20090131. If the return is for a quarter (Mar, June, Sept, Dec), enter the ending date of the quarter. For example: March 2009 should be entered as 20090331. This is a Required field.

Period (Column 3) – Enter M for a Monthly return (Jan, Feb, Apr, May, July, Aug, Oct, Nov) or enter Q for a Quarterly return (Mar, June, Sept, Dec). This is a Required field.

Return Type (Column 4) – Enter return type for the period being filed. Enter A6 for a Monthly return or A1 for a Quarterly return. This is a Required field.

Number of Employees (Column 5) – Enter the number of employees from whose wages Alabama income tax was withheld during the reporting period. This is a Required field.

Tax Withheld Amt (Column 6) – Enter the total amount of tax withheld, in dollars and cents (include decimal), for the reporting period. If a monthly report is being filed this amount will only be the amount of tax withheld for the month. If a quarterly report is being filed this amount will include all withholding tax for the three months of the quarter. This is a Required field.

Tax Remitted First 2 Months (Column 7) – Only enter an amount in this field if a Quarterly report (A1) is being filed. Enter the total tax previously remitted, in dollars and cents (include decimal), for the first two months of the quarter being reported. Enter zero (0.00) if no tax was previously remitted. This is a Required field for filing Form A1.

Credit for Overpayment (Column 8) – If a credit is due, enter the amount of overpayment, in dollars and cents (include decimal). The credit amount cannot exceed the amount of the tax due on this return. Leave this field blank if you do not have an overpayment and are not entitled to a credit. Note: If you are claiming a credit for an overpayment, you must also enter the period of the overpayment, the amount of tax withheld and the amount of tax remitted reflecting the overpayment amount in the applicable columns 12 – 23.

Amount of Tax Due (Column 9) – Enter the total amount of tax due for this return, in dollars and cents (include decimal). This is a Required field.

Final Return (Column 10) – Enter N if this is not a final return. Only enter Y if this is a final return, you have discontinued withholding Alabama income tax, and you want to close the account. This is a Required field.

Final Rpt Last Payment (Column 11) – If this is a final return, you entered Y in column 10, you must enter the date when you discontinued withholding Alabama income tax. Leave this field blank if this is not a final return. This is Required if you entered Y in Final Return column 10.

Prior Year Credit (Column 12) – Enter the year of the credit if an overpayment occurred in a prior year. Leave blank if no overpayment occurred in a prior year.

Prior Year Withheld Amt (Column 13) – Enter the prior year tax withheld amount, in dollars and cents (include decimal), for the year in which the credit occurred. Leave blank if no overpayment occurred in a prior year.

Prior Year Paid Amt (Column 14) – Enter the amount of withholding tax paid, in dollars and cents (include decimal), for the year in which the credit occurred. Leave blank if no overpayment occurred in a prior year.

Qtr 1 Current Year Credit (Column 15) – Enter the date of the last month of the first quarter. The format for this field is MMCCYY = month month century century year year. Leave blank if no overpayment occurred in this period.

Qtr 1 Current Year Withheld Amt (Column 16) – Enter the actual amount of tax withheld, in dollars and cents (include decimal), during the first quarter of the current year if an overpayment occurred during this period. Leave blank if there was no overpayment in this period.

Qtr 1 Current Year Paid Amt (Column 17) – Enter the amount of tax paid, in dollars and cents (include decimal), during the first quarter of the current year if an overpayment occurred during this period. Leave blank if there was no overpayment in this period.

Qtr 2 Current Year Credit (Column 18) – Enter the date of the last month of the second quarter. The format for this field is MMCCYY = month month century century year year. Leave blank if no overpayment occurred in this period.

Qtr 2 Current Year Withheld Amt (Column 19) – Enter the actual amount of tax withheld, in dollars and cents (include decimal), during the second quarter of the current year if an overpayment occurred during this period. Leave blank if there was no overpayment in this period.

Qtr 2 Current Year Paid Amt (Column 20) – Enter the amount of tax paid, in dollars and cents (include decimal), during the second quarter of the current year if an overpayment occurred during this period. Leave blank if there was no overpayment in this period.

Qtr 3 Current Year Credit (Column 21) – Enter the date of the last month of the third quarter. The format for this field is MMCCYY = month month century century year year. Leave blank if no overpayment occurred in this period.

Qtr 3 Current Year Withheld Amt (Column 22) – Enter the actual amount of tax withheld, in dollars and cents (include decimal), during the third quarter of the current year if an overpayment occurred during this period. Leave blank if there was no overpayment in this period.

Qtr 3 Current Year Paid Amt (Column 23) – Enter the amount of tax paid, in dollars and cents (include decimal), during the third quarter of the current year if an overpayment occurred during this period. Leave blank if there was no overpayment in this period.

Payment Method (Column 24) – Enter payment method as E for EFT Debit then enter the bank account type in column 32, bank routing number in column 33, and bank account number in column 34. Enter V for EFT Credit then initiate the payment through your banking institute. If no tax is due, enter payment method as Z. This is a Required field.

Funding Source (Column 25) – If this payment will be funded by money received from a financial institution located outside of the territorial jurisdiction of the United States enter Y for yes. If not, enter N for no. This is a Required field for EFT Debit payments.

Company’s Address (Column 26) – If you entered Y in Funding Source (Column 25), you must enter the mailing address of the company being filed.

Company’s City (Column 27) – If you entered Y in Funding Source (Column 25), you must enter the city of the company being filed.

Company’s State (Column 28) – If you entered Y in Funding Source (Column 25), you must enter the state of the company being filed.

Company’s Zip (Column 29) – If you entered Y in Funding Source (Column 25), you must enter the 5 digit zip code of the company being filed.

Company’s Zip + 4 (Column 30) – If you entered Y in Funding Source (Column 25), you must enter the 4 digit zip code extension of the company being filed.

Payment Date (Column 31) –Enter the date of your payment. You cannot enter a date prior to today’s date. This is a Required field for EFT Debit payments.

Bank Acct Type (Column 32) – If the payment method is E (EFT Debit), enter the bank account type. Leave blank if the Payment Method in column 24 is V or Z. This is a Required field for EFT Debit payments.

Bank Acct Type (Column 33) – If payment method is E (EFT Debit), – enter the bank routing number (9 characters). Leave blank if the Payment Method in column 24 is V or Z. This is a Required field for EFT Debit payments.

Bank Acct Number (Column 34) – If payment method is E (EFT Debit), enter the bank account number (3 – 17 characters) . Leave blank if the Payment Method in column 24 is V or Z. This is a Required field for EFT Debit payments.
