Income Tax Calculated/2-D Barcode Forms FAQs

All forms must be printed and mailed to address listed on the form.

Completing the form online is not suitable for tablets, phones (mobile devices) or Mac.

Some forms have been enhanced to complete calculations.  Input your data prior to printing the form or error message will pop up.

If you choose to use the fillable form, please do not handwrite any other data on the form other than your signature.  Your information will be included in the 2-D barcode.

Helpful Tips

  • Click in the first field where you want to begin entering data and input the information.
  • Use the tab key or mouse to move to the next input field.
  • Yellow fields are auto-calculating, do not enter data in yellow fields.
  • Enter SSN, FEIN or Withholding numbers without the dashes.
  • If you do not input required data or valid data format in the field, a popup error message will continue to display.

How do I save the form?

Click “Save Form.” The form may save in the downloads folder as a html file on your computer with a preset name (FormName-Tax Year-Date-Time). Example: 20f40.pdf-2020-6-28-12_9.html. Depends on how your download option set up, your file may save in a folder other than the download folder.

How do I reopen a saved file?

  • Locate the file on your computer (Where you save the file it maybe in the download folder of your local hard drive (C:).
  • Click the file name to open
  • File will open in your default browser

Notes:

  • If you select “Save Form” multiple times throughout your session the form will save a new file or multiple files each time
  • You must be connected to the internet to complete or edit the form
  • If you close or refresh your browser before saved the form, your input data will be lost.

 

 

How do I print the completed form?

Click the green “Print” button to print the completed form.  Once the print window opens you may be able to select specific pages or a print range.

Note:

  • Please set to print out the best quality.
  • Low quality print out may delay the processing of your return.

 

How do I clear the form?

Click the “Reset” button on page 1 to clear ALL the data on the entire form.

Click the “Reset Page” button on each page to clear the data on that page ONLY.
